A shift is taking place in healthcare today. For the first time, AI is not just refining traditional medical practices, it is disrupting and reshaping the very foundations of how we think about data, patient care, and medical decision-making, compelling us to rethink everything, from how we collect and interpret data to how we diagnose, treat, and engage with patients.

This publication presents the proceedings of MEDINFO 2025, the 20th World Congress on Medical and Health Informatics, Held from 9 to 13 August in Taipei, Taiwan. This biennial congress serves as a cornerstone for the global health informatics community, bringing together academics, healthcare professionals, policymakers and industry leaders to advance the science, application and governance of digital health worldwide. The theme of the 2025 conference is Healthcare Smart x Medicine Deep, reflecting the shift taking place in healthcare today. A total of 1,039 submissions were received for the conference, and after a rigorous review process involving 573 expert reviewers, 277 full papers, 34 student papers, 231 posters and 36 student posters were accepted for presentation at the conference and inclusion in the proceedings. They are grouped around 5 Tracks: information and knowledge management; quality, safety and outcomes; health data science and artificial intelligence; human, organizational and social aspects; and global health informatics

Showcasing global research, this proceedings highlights the important role of biomedical and health informatics in shaping the future of healthcare and wellbeing worldwide, and will be of interest to all those working in healthcare today.
